How to Submit a DS-64 Application Form

Now that your form has been completed, you can now submit the document to be processed. The application process is the same no matter whether you’re reporting a lost or stolen passport book, passport card or both at the same time. In all cases, you will have to send the required documents by mail to a National Passport Processing Centre. In order to complete your application, simply follow these steps:

Download and Print the Form We’ve Sent You by Email

  • Download the completed DS-64 application form that we have sent you by email

  • In order to correctly open your form, you will need to use Adobe Acrobat Reader

  • Print your application form and follow the rest of the instructions below

Remember: You should not sign your DS-64 form.

Calculate the necessary passport costs and fees and discover the different payment methods to pay the government processing fees.

Additional Supporting Documentation (if applicable)

If you have a police report, please, submit a copy with the rest of the documentation.

If you have changed your name since acquiring your most recent passport you will need to include supporting documents as evidence. 

Suitable examples include:

  • Marriage certificates

  • Divorce decrees

  • Court-ordered name change documents

Note:

If you are due to travel within 2 weeks you can make an appointment at a Passport Agency.

The automated appointment system is available 24/7.

Be aware however, this service can only be used if you have to travel urgently within 3 weeks or for life or death emergencies.
