Proof of Name Change Requirements
U.S. passports must always reflect an individual’s current legal name. If your name has changed due to marriage, divorce, or a legal name change, you are required to submit documentation supporting the change when applying for a new or updated passport.
Acceptable Documents (Submit Any One):
To update your name on a passport, please provide one of the following original documents as proof of name change:
Original Marriage Certificate
Original Divorce Decree
Original Court Order for Legal Name Change
