How to Submit a DS-11 Application Form

Now that your DS-11 form has been completed, you can now submit your documentation to be processed. In order to do this, you will need to present your application in person at a Passport Acceptance Facility or Passport Agency. The application process is the same no matter whether you’re applying for a passport book, passport card or both simultaneously. To complete your application for a U.S. passport, simply follow these steps:

Download and Print the Form We’ve Sent You by Email

  • Download the completed DS-11 application form that we have sent you by email

  • In order to correctly open your form, you will need to use Adobe Acrobat Reader

  • Print your application form and follow the rest of the instructions below

Remember: You should not sign your DS-11 form.

Documents to Submit with Your Forms

One Recent Passport Photo

  • Attach one photograph of yourself to the application. The photo must meet specific requirements to be accepted.

Passport Fees

  • You must include the Government processing fees for your passport in the form of a personal cheque or money order.

Proof of U.S Citizenship

Depending on your circumstances, you will need one or more of the following pieces of citizenship evidence:

  • A Birth Certificate

  • A Consular Report of Birth Abroad

  • A Certificate of Naturalization

  • A Certificate of Citizenship

  • Your current or previous U.S Passport

Proof of Identity

You must provide proof of identity in order to process your application. 

Commonly accepted forms of ID include:

  • A Driver’s License

  • A Certification of Naturalization

  • A Certificate of Citizenship
