How to Submit a DS-82 Application Form
Now that your form has been completed, you can now submit the document to be processed. The application process is the same no matter whether you’re applying for a passport book, passport card or both at the same time. In all cases, you will have to send the required documents by mail to a National Passport Processing Centre. In order to do so, simply follow these steps:

Remember: You must SIGN and DATE your DS-82 forms before mailing your documents.
Documents to Submit with Your Forms
One Recent Passport Photo
Attach one photograph of yourself to the application. The photo must meet specific requirements to be accepted.
Passport Fees
You must include the Government processing fees for your passport in the form of a personal cheque or money order.
Your Most Recent Passport and/or Card
You must send your most recent passport book and/or card with your application. These should be undamaged and must have been issued to you above the age of 16 and within the last 15 years.
Your old passport book and/or card will be returned to you. However, these usually arrive in a separate mailing to your newly issued passport.
Additional Supporting Documentation (if applicable)
If you have a police report, please, submit a copy with the rest of the documentation. If you have changed your name since acquiring your most recent passport you will need to include supporting documents as evidence.
Suitable examples include: Marriage certificates, Divorce decrees, Court-ordered name change documents
Where to Submit Your Form
Your application form and relevant documents must be mailed via a trackable USPS delivery to an applicable address listed below.
Remember: Use an envelope big enough to fit the application forms without folding them.

If you are due to travel within 2 weeks you can make an appointment at a Passport Agency.

The automated appointment system is available 24/7.
Be aware however, this service can only be used if you have to travel urgently within 3 weeks or for life or death emergencies.
